Common Clinical Cardiac Biomarkers

Cardiac Troponin T (cTnT)

cTnT is a highly sensitive and specific biomarker for myocardial injury. During acute myocardial infarction (AMI), degradation of myocardial thin filaments leads to the release of cTnT. Serum levels typically rise within 2–8 hours, peak at 12–24 hours, and remain elevated for approximately 14 days.

Cardiac Troponin I (cTnI)

cTnI is widely regarded as the “gold standard” biomarker for AMI diagnosis. As a cardiac-specific antigen, it demonstrates excellent specificity for myocardial injury. cTnI levels increase within 3–5 hours after cardiac injury, peak at 12–24 hours, and generally return to normal within 5–10 days.

Myoglobin (MYO)

MYO is an early biomarker for acute myocardial infarction. Abundantly present in cardiomyocytes, myoglobin is rapidly released after myocardial necrosis, with serum levels increasing within 1–2 hours and peaking at 6–9 hours, offering excellent sensitivity for early detection.

N-terminal pro-B-type Natriuretic Peptide (NT-proBNP)

NT-proBNP is an important biomarker for heart failure. It is primarily secreted by the left ventricle in response to increased cardiac pressure and volume overload. NT-proBNP is widely used for heart failure diagnosis, risk stratification, prognosis evaluation, and clinical decision support.

D-Dimer

D-Dimer is a sensitive biomarker for acute coronary syndrome (ACS). It is a degradation product generated when cross-linked fibrin is broken down by the fibrinolytic system. D-Dimer levels are clinically significant for thrombosis diagnosis, therapeutic evaluation, and prognosis assessment.

Creatine Kinase-MB (CK-MB)

CK-MB is one of the most valuable enzymatic biomarkers for acute myocardial infarction. Primarily distributed in myocardial tissue, CK-MB levels remain extremely low in healthy individuals but rise significantly following myocardial injury.

Heart-type Fatty Acid-Binding Protein (hFABP)

hFABP is a novel biomarker for myocardial injury. Detectable within 20 minutes after myocardial damage and peaking at 3–4 hours, hFABP serves as an ultra-early indicator of cardiac injury.

Soluble Suppression of Tumorigenicity 2 (sST2)

sST2 is an independent predictor of mortality risk and a biomarker of myocardial fibrosis. As a member of the interleukin-1 receptor family, sST2 testing is minimally affected by age, renal function, gender, or BMI, offering superior diagnostic and prognostic value in heart failure patients with renal insufficiency compared with natriuretic peptides.
